Cape Cod Health Care introduces new family care model

Hass Family Medicine opens in Hyannis

Cape Cod Healthcare is pleased to announce the establishment of a new model of family medicine, with the opening this month of Hass Family Medicine at 130 North Street in Hyannis.

This unique clinic model will be dedicated to improving access to primary care by providing a family practice clinic that offers early morning, evening and Saturday hours. Joel Hass, M.D. and his nurse practitioners, LeeAnn Giardino and Susan Hoover, will have clinic hours Monday through Friday, from 7 a.m. to 8 p.m. and Saturday from 9 a.m. to 2 p.m. In addition, their commitment to continuity of care will extend to Cape Cod Hospital as they will see their own hospitalized patients. During those hours when the clinic is closed, a member of the team will always be just a phone call away.

“Preventative health and coordination of patient care are both major goals and challenges in primary care. We believe our unique model addresses these challenges and achieves these goals.” - Dr. Joel Hass

“Preventative health and coordination of patient care are both major goals and challenges in primary care,” said Dr. Hass. “We believe our unique model addresses these challenges and achieves these goals.” The practice will be part of Medical Affiliates of Cape Cod (MACC), CCHC’s employed physician group subsidiary.

Dr. Hass is a fully-licensed board certified family practice physician with 18 years practice experience. He earned his Doctor of Medicine degree from The Chicago Medical School in 1989. Dr. Hass completed his residency at Via Christi Hospital in Wichita, Kansas in 1992. He is moving to the Cape from Chicago for the sole purpose of starting this exciting new clinic model.

“Dr. Hass’ clinic will be the pilot site for implementation of the latest in electronic health record technology,” said Charles Hulse, Executive Director, MACC. “This will allow the team to access important clinical information, anytime of day, that can benefit the patient in an emergent situation, and eventually connect with all MACC specialists.”

“In addition,” added Dr. Hass, “each patient will have a preventive health profile that the clinic will use to help patients arrange important screening tests such as routine health exams, mammograms, colonoscopies, vaccinations and other important health maintenance items.”

Please contact the clinic or schedule an appointment now by calling the 24-hour number 508 957-6500. The office fax number is 508 957-6590.
