WHO’S YOUR AGENT?  

October 13th 10:30 A.M. Part I: “Getting Started” and October 20th  10:30 A.M. Part II: “Next Steps”. Join Betsey Moehlich and Martha Byron for a two-part presentation on designating a health care agent and proxy. This program will take you through this process and help you begin talking to your family and loved ones about your health care wishes. The model presented is consistent with what is used at Cape Cod Health Care. Even if you already have a healthy care proxy, you will benefit from this class.
